This survey is meant to be anonymous. You may send it to Jubilee Farm, 229 West Snoqualmie River Road NE, Carnation WA 98014. Or, if you don't care about anonymity, you can return it electronically.

To identify comments that may pertain to one phase of our CSA operation we would ask you to complete the following:

1. Do you normally pick up your produce at the farm, or do you have your box delivered?
